| Objective 1: By 2017, increase participation of Williamson County providers in the Health Information Exchange (HIE) from 6 to 10. |
Objective 2: By 2017, decrease the percentage of emergency department (ED) utilization for non-emergency needs by coordinated care across providers in Williamson County from 28.4% to 23.4%.

Objective 3: By 2017, develop electronic patient referral protocols among diverse care providers in Williamson County. | ||
